Disconnect the connector for one of the injectors and rerun cylinder balance test to see if it identifies the correct cylinder

Looking at your log your maf counts are very unstable. I see a maf count of 36 at 1204 rpm and a few rows later maf is 112 at 1208 and this is at a relatively stable idle well before the surging

You could also jump the iac to power and ground right at the battery. Just do it for a second. Rpms should shoot up pretty high if iac is working. Make sure you use correct polarity.

Using 42# injectors without tuning the stock A9L will probably result in starting issues. The ECU will still think it has 19# injectors and add way too much fuel at start up.

I don't see how just logging the maf voltage while driving will help you. You would only be able to log maf voltage vs. rpm without being able to see fueling error percentage. Does the maf voltage change when you rev the engine? You might also want to use a volt meter and see if what you measure at the maf is the same as you are datalogging.

With maf voltage at 1.3-1.4 volts at idle I would suspect a bad maf. Is it possible to reinstall your old maf?

A maf voltage of 1.3 to 1.4 at idle sounds really high. I have the same maf and mine is .8-.9 at 850rpm

A lot of aftermarket throttlebodies make a whistling noise. I have an Accufab TB and have no noise.

You may want to try putting a screen before and after the maf to help with turbulence. Also the slot style mafs are less sensitive to turbulence.

If your engine flows over 1,750 kg/hr for EEC-V or 2,350 kg/hr for EEC-IV you will have to scale airflow.

What sensors are you using? Cheap sensors don't last. Just because the wire harness is new does not mean it's good. Could still be a wiring issue.

Here is my tune with the PMAS MH95-3000. Just copy my maf transfer to your tune. You will probably still need to tweak it some but should be pretty close.

I had a hell of a time getting my QH connected to my windows 7 laptop. My problem turned out to be I had to actually enter the exact file name and location for the driver in the box where windows asks for the drivers software location. Browsing and clicking on the file folder did not work for me.

FN1321 is how much transient fuel and FN1322 is how long it lasts. In both tables increasing the value increases the amount of transient fuel.
