I noticed that there was a factory set RPM limit at 4000 when the gear is on P or N.
I was wondering if you were aware of any other RPM limit set from factory.

I haven't tuned any 19 express vans to be certain but GM has been using ignition limiters for some time now. If the whole bank drops out then its probably hitting a staged limited and continued rpm or vehicle speed will kill the other if I had to guess. Back in the day they would just kill all igni...

On the ford coyotes the relearn is usually not completed due to the software asking for the relearn procedure not being coded correctly Not sure if the GM your experiencing it on would be similar.
